Endocrine therapy runs five to ten years, and for a woman in her mid-thirties who hasn't yet had children, that doesn't just reshape her treatment — it reshapes her life.

The POSITIVE trial was built around that reality. Led by the International Breast Cancer Study Group and Ann Partridge, MD, of Dana-Farber Cancer Institute, the study enrolled 516 premenopausal women with HR+, HER2-negative early breast cancer who wanted to pursue pregnancy. The protocol allowed them to complete 18 to 30 months of endocrine therapy, then pause for up to two years to conceive, deliver, and breastfeed before resuming treatment. The central question: would that pause cost them their lives?

First results in 2023 were reassuring. Now, with a median follow-up of 71 months, updated data presented at the 43rd Annual Miami Breast Cancer Conference have deepened that reassurance considerably. Of 497 evaluable participants, 76% became pregnant at least once, producing 589 pregnancies and 440 live births. Obstetric complication rates fell squarely within population norms for women of comparable age.

The oncologic picture held equally steady. Compared against a matched cohort from the SOFT and TEXT trials, breast cancer–free interval events were 12.3% in the POSITIVE group versus 13.2% in controls, with a hazard ratio of 0.88. Distant recurrence–free interval events were 6.2% versus 8.3%, a hazard ratio of 0.67. A landmark analysis comparing participants who conceived within 18 months to those who did not found no statistically significant difference in breast cancer–free interval — and the hazard ratio favored the women who had gotten pregnant, at 0.64.

Assisted reproductive technology was used by 43% of participants, with embryo transfer more than doubling the odds of achieving pregnancy and no signal of oncologic harm. Adherence to resuming endocrine therapy after delivery was notably strong at 72%, comparing favorably to general population data where roughly half of young patients discontinue treatment within five years.
